Holiday Club Interest Form

This form is an expression of interest and is not a confirmation of a place on a SEND HAF activity. This form is to be completed so that your child’s needs are correctly assessed and matched with our resources and provisions. The HAF Evoucher application date will be used to prioritise acceptance onto the activity. The Evoucher platform will be used to confirm your registration after we have successfully matched the needs to the provisions.
